Registration Form
Completing the form below is the first step in registering your child with Dare to Be Different Youth Empowerment Organization. This form must be completed by a parent or legal guardian.
Once your form has been submitted, our team will begin the registration review process. You will receive a welcome email within 2 business days with additional information regarding our programs, next steps, and any required documents.
